At Aurora, we serve individuals across all counties in Minnesota through personalized, home and community-based services designed to promote dignity, independence, and peace of mind. If you know someone who is currently on a waiver and enrolled in Medicaid, they may be eligible for our services.

How It Works
Submit a Referral using the form below.
Our team will follow up with the referred individual or their representative to confirm eligibility and complete the intake process.
Once we receive the service authorization, you receive a $500 referral bonus — no strings attached.
Please note: Referral bonuses are paid after Aurora is officially authorized to provide services for the referred client. Limit one bonus per client.
